  HOW TO REQUEST AN OFFICIAL TRANSCRIPT

    Note: Once a transcript request has been submitted it cannot be canceled or changed.

  1. Submit a written request by mail, in person or by fax at (803) 777-6349. (To avoid duplicate charges, contact our office prior to resubmitting a previously faxed request.)

    OR

  2. Submit your request online at VIP. For more information on requesting official transcripts using VIP, click here.

    *Must have attended USC from 1977 to present and have a valid PIN.

    Note: Transcripts requested on VIP are mailed and are not available for pick-up.

  • Transcripts are $8.00 (eight) per copy. This fee is NON-REFUNDABLE.
  • Payment by check, money order or credit card must accompany your request. Please make checks and money orders payable to USC.
  • Transcripts will not be issued for persons who have an outstanding balance or unfulfilled obligation to the university.
  • Only complete official USC transcripts are issued and will include all USC campuses.
  • USC does not issue unofficial transcripts.
  • USC does not fax transcripts.
  • You must contact our office for pre-paid mailing services (Federal Express, Express, etc...). Requests for these services cannot be requested on-line through VIP.
    Instructions for Prepaid Mailing Services

The Family Educational Rights and Privacy Act of 1974 requires that all transcript requests be in writing, signed and dated by the person to whom the record belongs.

TO SUBMIT A WRITTEN REQUEST

Download our official Transcript Request Form. Adobe Acrobat Reader is required to view and print this form. If you do not already have Acrobat Reader, click on the Acrobat Reader link and download your free copy.

All written requests must include the following information:
  • Your Full Name as it appears on your records and any other name(s) used while enrolled.
  • Student Number.
  • First and Last Terms of Enrollment.
  • Recipient Address to which your transcript is to be mailed.
  • Your Return Address, Telephone Number and Email Address.
  • Signature and the Date. Your request will not be processed without your signature.

You may fax your request with a valid Visa or MasterCard number for payment to our office at (803) 777-6349 (to avoid duplicate charges contact our office prior to resubmitting a previously faxed request) or come by our office, third floor Petigru Building at the corner of Greene and Pickens street. You may also mail in your request along with the $8.00 processing fee to the address below.

Name Change Request Form

To change the name on your academic record, you must present to the University Registrar’s Office a Name Change Request Form and copies of legal documents, which indicate the name change.
